Prof. Dr. Afzal performed the first zero-contrast angioplasty in Bangladesh

Zero-contrast angioplasty was done, for the first time in Bangladesh, on 30 July 2019 in the National Institute of Cardiovascular Diseases (NICVD). Prof. Dr. Afzalur Rahman, the Director and Professor of NICVD, placed stents (rings) in heart vessels of a cardiac patient with kidney disease using this technic. After that, he performed more complex cases of zero-contrast PCI successfully with this newly introduced technic (by using intra-vascular ultrasound, IVUS and fractional flow reserve, FFR).

During conventional angioplasty, Intravenous contrast agent needs to be used which may be hazardous to the patients with kidney disease or allergy. This alternative technic has created a ray of hope for this subset of patients. This zero-contrast technology has been introduced in a few developed countries of the world, and use of this method by Dr. Afzal, is in fact, the first one of its kind in South Asia.

In recent decades, the average life expectancy has increased significantly, along with this, the number of patients with diabetes and hypertension has also increased steadily. A major fraction of these patients simultaneously suffer from kidney disease. Here lies the importance of zero-contrast angioplasty. In this method, virtually no contrast agent is needed.

Introduction of this newly introduced technology involving IVUS and FFR will remain a milestone in the history of interventional cardiology in Bangladesh.

Prof. Afzal was born in a well-respected family at Baraigram, Nator. He graduated from Rajshahi Medical College in 1984, and obtained his MD Cardiology degree from NICVD in1992. He had Fellowship training in Intervention Cardiology from Hopital Charles Nicolle, University De Rouen, France under the pioneer interventional Cardiologist Prof. Alain Cribier in 2000.

He obtained PhD in Cardiology from Dhaka University in collaboration with University of Brimingham, USA in 2004. Dr. Afzal received FRCP from the Royal College of Physicians and Surgeons of Glasgow and the Royal College of Physicians of Edinburgh, UK. He earned the Fellowship from the American College of Cardiology (FACC).

In recognition of his outstanding performance and contribution in the field of Interventional Cardiology, Dr. Afzal received numerous appreciations and awards.

In 2007, he was rewarded as the Winner of the Most Challenging Case Around the World in percutaneous Transluminal Coronary Angioplasty (PTCA) stenting by TCT 2007 (Transcatheter Cardiovascular Therapeutics, USA).

In 2009, he received the Emerging Young Leadership Award Around the Globe, CRT-2009 in the field of Interventional cardiology from the United States (The Cardiovascular Research Institute at Washington Hospital Center and Cardiovascular Revascularization Therapies (CRT) 2009).

Prof. Afzalur Rahman is the elected President of Bangladesh Society of Cardiovascular Intervention (BSCI), Chairman and Course Director of Bangla Interventional Therapeutic (BIT) and International Governor of American College of Cardiology. He served as the Associate Director of the US-based Transcatheter Cardiovascular Therapeutics (TCT-2016).

Dr. Afzal has been working relentlessly with wisdom and professionalism as a doctor for the patients, as a teacher for the students, and as a leader for professionals. Consequent upon this, and with the effort of others, Cardiology in Bangladesh is already at a prestigious level in the world, and is progressing steadily.

Dr. Afzal, the iconic personality of Modern Cardiology in Bangladesh, will continue to work for the nation and the country.
