Combating TB

World Tuberculosis Day was observed in the country as elsewhere in the world yesterday. This year the slogan of the day was “It's the right time to make a pledge to create TB-free Bangladesh”. It is disturbing to note that although tuberculosis (TB) infections have been a concern for Bangladesh for quite a long time, screening facilities are still not available in every upazila. According to the Global Tuberculosis Report 2018 of World Health Organization, Bangladesh is among the top 30 high burden countries for TB and multi-drug resistant TB (MDR-TB).

In this situation the latest tool that can successfully detect TB on a molecular level and drug resistance -- Gene Xpert -- have not reached even half of the county's medical complexes. Despite achieving a remarkable success in detection and treatment of tuberculosis, it is still a major public health problem in Bangladesh. According to a renowned chest specialist, there are at least five TB patients in an assembly of 100 people. That is one in every 20 people is suffering from TB. So, there is no room for expressing complacency regarding the state of TB disease in the country.

In 2017, about 558,000 people worldwide developed TB that was resistant to rifampicin, the most effective firstline drug, and of these, 82 per cent had MDR-TB. Drug resistance (DR) occurs when anti-TB medicines are used inappropriately through incorrect prescription by health care providers, poor quality drugs, and end treatment prematurely. Living in congested rooms, damp conditions, and malnutrition were major reasons behind TB. The cost of treatment for a DR-TB patient is 30 times higher than a normal TB patient. The duration of treatment is also longer.

Taking medicines continuously in right dose for a certain period of time as advised by the physician may cure the disease. What is needed is the proper treatment after early detection of the disease. The government must put in the required efforts to stamp out tuberculosis. A vigorous search programme must be launched to find the tuberculosis patients who are still not under the coverage of treatment.  Massive awareness should be generated among the people so that the people suffering from the disease may undergo treatment at the state-run TB hospitals and clinics.
