15-20 cm long partly exocrine (digestive juice) and partly endocrine (insulin) Pancreas means all flesh (Pan=all, Kreas=flesh) which lies behind peritoneum i.e stomach on the back wall of the abdomen and extends from second part of duodenum two spleens. It weighs 90 gm, lies at the level of L1 and L2 vertebra. It has a head, neck, body and tail. The bile duct and pancreatic duct enter second part of duodenum and joint together forming Hepato-pancreatic ampulla of Vater and open at the summit of major duodenal papilla, 8-10 cm distal to pylorus. Parasympathetic nerves, cholesystokinin-pancreaozymin of duodenum play part in secretion. From Islets of Langerhans Alpha cells produce Glucagon and Beta cells produce insulin.

When organ is inflamed it is called Pancreatitis which affects 10-20% of one lac population. Abdominal pain worst after eating radiating to back, fever, rapid pulse, nausea, vomiting and tenderness marks the onset of disease. Severe cases are life threatening and digestive enzyme starts digesting pancreas itself due to activation of trypsinogen. It should be differentiated from coronary occlusion, intestinal obstruction, perforated peptic ulcers, gallstone colics, leaking dissecting aortic aneurysm etc.

Causes are alcohol, gallstone, idiopathic, and rarely due to trauma, surgery (abdominal cardio-pulmonary bypass, operation on stomach, gall bladder, spleen, organ transplant etc), infections (mumps, coxsackie virus), drugs (azothioprine, thiazide diuratics) etc. Diagnosis is made by history, physical examination, ultrasonogram (pancreatic swelling), rise of serum amylase, CT Scan etc. In chronic pancreatitis there is fibrosis and destruction of acini and functional failure. There may occure in addition to other symptoms fatty stool, steatorrhoea, diabetes. We must take care of this powerful of dual functioning organ.
