Family medicine education in Bangladesh

“Family Medicine is the medical specialty that provides continuing and comprehensive health care for the individual and the family. It is the specialty in breadth that integrates the biological, clinical and behavioral sciences. The scope of family practice encompasses all ages, both sexes, each organ system and disease entity.” -AAFP

Family medicine education in Bangladesh

Family medicine is based on the knowledge of the patient in the context of the family and the community, emphasizing disease prevention & health promotion. (patient centered/based not only the disease centered.)

Family/General practice does not seem to be understood or given due recognition either by communities, policy makers and governments, or even the medical profession.

Terminology

The term “family medicine” is used in many European and Asian countries (Bangladesh) & USA, instead of “general medicine” or “general practice”.

In Bangladesh, majority of the people are getting services from locally trained health care and CAM providers.

Why family medicine?

Easy approach/First contact or line doctor

Always available/One-Stop medical service

Services for all (ages/sexes/ethnics/religions etc)

Patient (disease + person  ) centered approach

Management/treatment for any organ/disease ( acute or chronic)

Patients have/get psycho-social support

FM ensures curative, preventive and rehabilitative services

 The only specialty that provides accessible, cost-beneficial and equitable health care to a nation.

“Only family physicians ( trained in FM) can provide accessible, cost-beneficial, and equitable healthcare.”

Family Medicine Education:

Family Physician’s holding degree in different countries:

 a) USA-- An M.D. or a D.O. after 3 years FM residency. Requires ABFM certificate exam. every 7 to 10 years through ongoing process of *CME, *medical knowledge review,*patient care oversight chart audits,*practice based learning through quality improvement project.

b) Sweden– 5 years working certification with a tutor.

c) India --- 3 year FM residency to have MD or DNB

Family medicine and family physicians play a very important role in the healthcare system of a country.  

FM courses in Bangladesh

Organizations working and awarding post graduate degrees :

a) BPMPA (BCGP):                1985 --FCGP

b) BAFP (BIFMR-USTC):        2000  -- FMD

c) BCPS-BSMMU (GOVT.) : 2004 --- MCPS & FCPS

d) RCGPI : 2007     ---MRCGP

Doctors with FM degree:

At present Bangladesh has 220 doctors with FMD (Family Medicine) degree throughout the country.

The demand for FMD and FCGP courses are seeing a major increase. More doctors are enrolling as they can increase their acumen in modern/updated medicine for managing & treating their patients.

Seventy eight doc with MCPS in FM

In 2012 session (12th batch) 39 doctors enrolled in FMD course.

Challenges

Family medicine is still a new subject in Bangladesh.

Community people, even doctors are confused.

Our government yet to recognize the education in family medicine specialty.

Medical colleges should include Family Medicine in its curriculum.
